These are what I consider key papers in the development of ophthalmology.
These papers have changed the understanding and management of eye conditions.
- This key paper in the treatment of age related macular degeneration (AMD) ushered in intravitreal delivery of anti-angiogenic / anti-VEGF therapy. Although the treatment effect in reducing visual loss was similar to photodynamic therapy, it began the modern approach to AMD treatment
- This landmark paper published in 2002 showed that reducing the intraocular pressure reduced the progression of primary open angle glaucoma. Reducing intraocular pressure by medications, laser or surgery is the mainstay treatment of primary open angle glaucoma today
- This landmark paper defined the treatment approach to acute postoperative endophthalmitis. Vitrectomy was beneficial for cases with visual acuity of light perception only. When intravitreal injections are given, systemic antibiotics had no additional benefit. Intravitreal antibiotics remains the mainstay of treatment for postoperative endophthalmitis today.
- This trial showed that intensive diabetic therapy delayed the onset and slowed the progression of diabetic retinopathy
- The defining paper on the treatment of acute demyelinating optic neuritis. Intravenous steroids followed by oral steroids resulted in accelerated visual recovery
- Key paper in highlighting the benefit of Pan Retinal photocoagulation in reducing severe visual loss and progression of diabetic retinopathy
- Lincoff’s rules. Finding the retinal hole. The classic paper on finding the retinal break based on the configuration of the retinal detachment.
